The Signal
Every project meeting produces actions. Most of them never make it into the right register.
Not because nobody cared. Because someone said “can you get that to me by Thursday?” at 11:47 while two other conversations were happening, and by the time the meeting ended it was a mental note in three different people’s heads, none quite the same.
The failure isn’t commitment. It’s capture.
The Noise
“AI can’t understand the context of what was said in the meeting.”
It doesn’t need to.
It doesn’t need to know the history between the delivery lead and the sponsor. It doesn’t need to understand the politics behind why the risk was only mentioned once and quickly moved on from. It doesn’t need the nuance you carry from eighteen months on this project.
It just needs to know what was said. And it’s very good at extracting what was said.
You supply the judgement - whether an action was real or just enthusiasm, whether a risk is as serious as it sounds, whether the decision actually carries authority. That’s your job. The extraction is not.
The Tool

action item - including ones mentioned casually, not just formally
assigned ones.
For each action, produce a row in this table:

- Owner: use the name stated. If inferred, mark as (inferred).
If unknown, mark as [UNKNOWN].
- Due Date: use the date stated. If inferred, state your inference.
If absent, write [NOT STATED].
- Priority: High / Medium / Low
- Confidence: HIGH (explicit) / MEDIUM (inferred) / LOW (uncertain)
- Source Quote: the phrase that triggered this item

You’ll get a table. Some cells will say [UNKNOWN] or [NOT STATED]. That’s not a failure - that’s the prompt telling you which conversations to have before the week ends.
